By signing below, I confirm that I have read, understand, and agree to the following:
Voluntary Participation
I am voluntarily participating in dances, classes, social events, and other activities sponsored, managed, supported, or provided by The Oklahoma Swing Syndicate (TOSS) and hosted at Presbyterian Church in Brookside (collectively, “TOSS activities”).
Assumption of Risk
I understand that participation in TOSS activities involves inherent risks, including the risk of personal injury, illness, property damage, or death. I knowingly and voluntarily assume all such risks, whether caused in whole or in part by my own actions, the actions of others, or the ordinary negligence of TOSS or its affiliates, excluding gross negligence.
Release of Liability
I release and agree to hold harmless TOSS and its officers, directors, volunteers, instructors, affiliates, and hosting partners from any and all claims arising out of or related to my participation in TOSS activities.
I agree to indemnify and defend TOSS against any claims, including reasonable attorney fees, resulting from my own negligence, recklessness, or misconduct during any TOSS activity.
Emergency Medical Consent
I consent to receive emergency medical treatment if deemed necessary during a TOSS activity and understand that I am responsible for any resulting medical costs.
Photo and Media Consent
I grant permission, without compensation, for my image or likeness to be used in photographs or recordings taken at TOSS activities for promotional, educational, or archival purposes, including use on websites, social media, printed materials, and other media
